J-1 Student Intern

The J-1 Student Intern category requires submission of a written evaluation of the scholar's performance. A final evaluation is required at the end of their J-1 program at UCSD and, for programs lasting longer than 6 months, a midpoint evaluation is also required.

Prior to submitting this form, discuss the evaluation requirement with your faculty sponsor  and make sure they are aware they will be receiving an email request to evaluate your internship experience.

Choose the evaluation type, based on the length of your program and which evaluation is being submitted.

Please enter the name of your faculty sponsor who is able to best evaluate your program. This should be your PI, as they will be aware of your program and progress here at UC San Diego. Typically this will be the faculty member who is supervising your program.

Please do not list ISEO staff or department administrators.

Supervisor Evaluation Form

Once you initiate the evaluation, your faculty sponsor will be asked to answer several questions about and appraise your progress in the program.

Supervisors should be prepared to answer questions about the following:

  • Communication skills and working relationships
  • Dependability, Quality of Work, and Initiative
  • Accomplishments achieved during the program
  • Areas that can be improved upon
  • Any additional comments on strengths, skills, and performance
